If you work for a region, or with a healthcare provider from whom the region has contracted healthcare services, you can use the form on this page to request personal data or other documents.

If you work for a contracted healthcare provider, we check that the healthcare company has a contract with the region. We do this when processing your request for the disclosure of information or documents.

We disclose the information necessary for your processing after a request has been received and a secrecy review has been carried out. In order for tasks covered by secrecy provision to be disclosed, Arbetsförmedlingen needs valid consent from the jobseeker. If there is no consent, you need to submit it first and then make your request after three working days.

Matters received are processed in order of priority. You will usually receive a reply within four working days. The reply will be sent to the official postal address of the region or to the address registered by your employer on 1177, addressed to you as the requester.

Important to note: Do not use the form if your request concerns a person with protected identity, the protection of children, or national security. In such cases, contact us by phone.
